New Patient Appointment
Now accepting new patients!
A "New Patient" appointment is required to establish care with Jan Nelson. These appointments are 60 minutes in length and thoroughly review your health history and concerns.
A "New Patient" appointment may be required if you have not seen Jan for an extended period of time.

Pellet Therapy Appointment
Hormone pellet therapy is an in office procedure performed by Jan Nelson. The appointment is 30 minutes in length and is typically scheduled 3-6 months apart.
$350 - To receive 1 hormone

Cortisol Awakening Response
A home-collection test to identify adrenal hormone imbalances that affect health.
This is a take-home saliva test that is mailed into ZRT Laboratory per the instructions in the kit.
While the take home test is free from Jan's office, a payment directly to the laboratory is needed to obtain your results.

Follow Up Appointment
A "Follow Up" appointment is necessary to review labs, medications, and/or other concerns. These appointments are 30 minutes in length and targeted to a specific topic or need.

Semaglutide - For Weightloss
Semaglutide appointments are scheduled once monthly after your initial consultation. This monthly check-in allows you to meet with Jan (or her Medical Assistant) to review any weight changes, symptoms, and if the current prescription dosage needs to increase or decrease.

Sleep Balance
A home-collection test to identify hormone imbalances that affect sleep.

Although we are not accepting insurance, we are able to create an itemized invoice for you to bill back to your insurance provider.
